Bad Limit Fills

Today on the DAS platform I was setting sell limit orders.  And despite the fact that the price was easily above my sell price- it wouldn't sell my position- even after ~10 seconds of being greater than my limit.  I've certainly seen it hit the limit price exactly and not sell, but  I don't remember this happening before and it happened twice today. 

Any thoughts as to why?

I have always had terrible fills in SIM, even direct from DAS.
